Release Information for Mountain Kids Ministry
* Photo/Video Release Authorization - I grant to Mountain Christian Church and their legal representation all rights to use and publish
photos/videos/filming and sound recordings of my children. I understand that the photos, videos/filming, or sound recording may be used for,
but not limited to, advertising, web designs, printed materials, videos, sound recordings and all other creative medium. I release Mountain C
hristian Church and their representatives and assigns from all claims and liability relating to all creative media taken of my children
on the premises. The Parent/Legal Guardian consents to their child(ren) being the subject of the media set forth above.
* Medical Treatment Authorization - In the event of an emergency, I authorize first aid or medical treatment for this child and I release Mountain Christian Church from any
and all responsibility in connection therewith.
